We realize the service we are providing is only one aspect of our client’s support system. We work as a team with all of our clients other supports, both their natural supports such as family or friends and their professional supports — Intensive Case Managers, Crisis Intervention, Intensive Community Integration Services and other agencies or individuals recognized as members of the Service Support Team
Intake Process
Once a referral is received by MBHA, an appointment is made with the individual and the case manager (referral originator) to complete an intake. At this meeting, a preliminary assessment and preliminary service plan will be completed. We will also obtain needed demographic data and authorizations to coordinate services with other providers.
On Going Assessment and Consultation
MBHA considers service plans to be living documents — written records of a continuous process of assessment, goal development, implementation and review, and adjustment of services to reflect the individual’s progress. This on-going process is done in conjunction with the Service Support Team whose membership includes the individual, the case manager and other providers who support the client’s community tenure. MBHA meets with the Service Support Team a minimum of every 90 days, and attends all meetings related to our client or impacting our service provision.
